Field Staff RN/Case Manager

Job Description

Responsible for assessing patient/family/caregiver needs, planning for the care, implementing the Plan of Care, and evaluating the effectiveness of the care provided. Oversees the direction and coordination of a patient's care throughout their time in hospice. Two openings for Field Staff RN/Case Manager, based out of Belleville office. The position in the Northern Territories covers Madison, Clinton, Bond, Montgomery, Macoupin, Jersey, and Calhoun Counties. The position in the Southern Territories covers St. Clair, Washington, Monroe, Perry, and Randolph Counties.

Qualifications

Graduate of a state-approved school of nursing.

Associate Degree in Nursing or Diploma in Nursing.

Current Illinois nursing license (Registered Nurse)

Certification in hospice and palliative care preferred.

CPR certification preferred.

One year hospice experience preferred.

Excellent leadership, managerial ability, clinical assessment, organizational, and interpersonal relationship skills

Working knowledge of Medicare/Medicaid, hospice regulations, and third-party payor expectations.

Reliable means of transportation.
