Case Manager/QIDP

Job Description

To serve as an advocate/case manager to the clients of Bryan Manor; developing individualize programs of training and services to meet clients' specific needs/goals.  Oversees the active treatment plans and coordination of care with nursing staff and administrative staff, ensuring that care remains current and completed in accordance with all Federal, State, and local standards as well as the facility policies and procedures.

Qualifications

Graduate of an accredited college/university with a minimum of a bachelor's degree in the following: Social Work, Recreation, Special Education, or related human services field (OT, OTA, PT, PTA, RN, Dietician, Psychology, Speech Language Pathology, Audiology). One (1) year experience in working directly with individuals with intellectual or other developmental disabilities is preferred but not required.  Must possess a valid driver's license and good driving record.

Salary and Benefits

Full Time; Monday-Friday day shift/ no weekends; benefits: health and dental insurance, life insurance, paid time off, tuition reimbursement, retirement
