Assistant Case Manager/QIDP

Job Description

To assist the Case Manager/QIDP in duties as assigned and assisting in the services to meet the individual's personal goals, preference and wishes.  Assist the Case Manager/QIDP in the active treatment plan, ensuring that all planning and coordination is complete in accordance with all Federal, State and local standards as well as facility policies and procedures.

-Assist in the supervision and training of Direct Support Personnel in the correct implementation of individualized training programs and services to the residents and reviewing the process at intermittent times

- Assure that data is being properly collected, is correct and is kept up to date

- Revise programming when necessary and per the direction of the Case Manager/QIDP to ensure progress or prevention of regression

- Assure that there is continuity and appropriate joint programming with Developmental Training sites

- Develop an individual daily schedule for each assigned resident

- Escort individuals on medical appointments as requested.  Obtain information from providers relevant to the care and programming

- Obtain certification to drive facility vehicles, CPR/First Aid/AED and other required skills  as required

- Provide Assistant QIDP services for individuals on other's caseloads as needed

- Participate on committees as assigned

- Schedule and chair special staffing's as needed to address changes in the individual's status on a timely basis

- Ensure that all personal items including clothing, etc. are always purchased and avaliable

- Communicate with family, guardian, and other community service providers to ensure information is provided on a timely basis

- Monito the funds and expenditures of individuals to ensure saving and ability to purchase items that are needed and preferred by the individual and/or their guardian

- Review and investigate incident reports daily as assigned.  complete GER's and resolution in Therap

- Serve as a champion for the 10 Principles for Personal Leadership

- Willingness to work beyond normal business hours and in other positions temporarily, when necessary

- Assist in various other functions within the department as necessary and willingly accepts other duties asassigned

- Maintain confidentiality of all pertinent information of resident data as well as care information to assure resident rights are protected

- Must attend all in-service trainings/staff meetings when necessary

- Must be able to lift 50 lbs; be capable of bending, stooping, reaching and walking

Qualifications

Must be a graduate of an accredited college/university with a minimum of an associate degree in the following: Social Work, Recreation, Special Education or related human services field.

One (1) year experience working directly with individuals with intellectual or other developmental disabilities is preferred but not required

Valid Drivers license and good driving record is preferred
