APPLICATION FOR SCHOLARSHIP TO BE AWARDED TO A 2013

CENTRALIA HIGH SCHOOL GRADUATE BY THE CENTRALIA BUSINESS AND PROFESSIONAL WOMEN'S CLUB

QUALIFICATIONS

1.   Applicant must be a member of the Centralia High School graduating class of 2013.

2.   Applicant must have at least a 3.0 GPA through the Fall 2012 semester.

3.   Applicant must submit a transcript for all course work through the Fall 2012 semester WITH THIS APPLICATION.

4.   Recipient must provide a final transcript indicating having graduated and must provide evidence

of enrollment for at least 12 semester hours of credit for Fall 2013 before the scholarship funds are paid to the recipient's  college.

5.   All applications must be postmarked on or before February 28,2013.
